Welcome to the FMH Secure Service Site Registration!
This form will help you set up your username and password for the FMH Secure Service Site. The new fmh.com incorporates single user logins and passwords. Previously, one username and password was issued per agency code number. Now each user within the agency will have the ability to set up their own personal access using our automated self-registration process. As we move to this new functionality, it will require every user to set up a new username and password.
Self Registration Details
AGENT ACCOUNT SET UP: >> If you are an existing FMH agent and would like to set up an online account, please fill out the form above and submit your information. ** It is VERY IMPORTANT to provide your valid email address and tax id. This will be the key information that we use to match your identity to our database system. Your tax id is needed to provide the best level of security possible.
>> You will receive a confirmation email at the address provided above shortly after submitting the form. ** The email will include a link that you MUST click on to continue with the registration process. Clicking the link validates your email address.
>> After clicking the link and validating your email, you will see a confirmation screen that contains your temporary password. ** Please be sure to PRINT or make note of your new password. You will use the username that you selected and your new password to login to the secure site. You will be able to login immediately, but you WILL NOT be able to view any secured information until your request has been approved. See next step.
>> Once you have been approved, you will receive ANOTHER CONFIRMATION EMAIL letting you know that your request has been approved. The email will include a list outlining all agency code numbers you will have access to. **If any code numbers are missing from your approval listing, please contact the web team using the form under the contact us tab.
POLICYHOLDER ACCOUNT SET UP >> If you are an existing FMH policyholder and would like to set up an online account, please fill out the form above and submit your information. ** It is VERY IMPORTANT to provide your valid email address and tax id. This will be the key information that we use to match your identity to our database system. Your tax id is needed to provide the best level of security possible.
>> You will receive a confirmation email at the address provided above shortly after submitting the form. ** The email will include a link that you MUST click on to continue with the registration process. Clicking the link validates your email address and takes you to the policy number verification screen.
>> The link in the confirmation email will take you to the policy number verification screen. Please enter the policy number that is associated with your SSN/tax id. ** The policy number verification screen is another level of security that FMH uses to make sure that your SSN/tax id matches the policy number that you are requesting access to, ensuring the highest level of security possible.
>> After submitting your information, you will see a confirmation screen that contains your temporary password. ** Please be sure to PRINT or make note of your new password. You will use the username that you selected and your new password to login to the secure site. You will be able to login immediately, but you WILL NOT be able to view any secured information until your policy number has been matched with the provided tax id. See next step.
>> Once your policy number has been matched to your provided tax id, you will receive ANOTHER CONFIRMATION EMAIL letting you know that your account has been activated. **You will then have full access to your policy information 24 hours a day, 7 days a week.
